Blepharoplasty and the passage of time

The contour around the eye is the most fragile and delicate part of the face. It requires special attention because age, tiredness and stress leave marks which reveal the passage of time.

Blepharoplasty is a surgical procedure performed on the eyelids, whether functional or cosmetic, where the primary objective is to remodel the shape of the eyelids by remove excess skin and fat of the upper and lower eyelids. With this type of surgery we can remove the "bags" from the eyelids and the tired appearance aging has on the face.

This type of surgery does not cause remove wrinkles or crow's feet, nor does it correct fallen brows. However, there are several procedures that can be combined with blepharoplasty to address these problems. Blepharoplasty is an ambulatory procedure (or day surgery) and does not require hospitalization.

The recovery period is fairly quick. Swelling and bruising after surgery disappears between one and two weeks after surgery depending on the extent of surgery. The surgery lasts around 1 to 3 hours to complete. If you are interested in blepharoplasty, it is important to set up an appointment so that our Oculoplastic Specialist can perform a thorough evaluation and respond all pertinent questions surrounding the surgery, recovery and expectations.

Who is the best candidate to undergo blepharoplasty?

Blepharoplasty helps to improves one's appearance and self-confidence. However it is important to talk with the specialist and carefully review your expectations with the surgeon.

Ideal candidates are men and women that are physically healthy with psychological stability and real expectations. The majority of patients who will undergo blepharoplasty are over 35 years of age.

Who will benefit from phacoemulsification?

Most people suffering from cataracts will benefit from this procedure. Best results are obtained with early cataracts. It is preferable not to wait until the cataract is ripe and vision is excessively low since the cataract will be too hard for the phacoemulsifier. Surgery is indicated as soon as vision is not adequate for daily activities.

Frequent Questions

  • What experience do we have in Keratoconus and Intraocorneal Ring surgery?

Clínica de Ojos de Tijuana was first ophthalmology center in Mexico and one of the first Worldwide to use intracorneal ring 10 years ago for keratoconus treatment.

Today, we are a referral center for keratoconus patients in Mexico and the southwest United States for the treatment of this disease.

We have performed this surgery in thousands of keratoconus patients with very good results.

  • What are the objectives with intracorneal ring surgery?

The main goal of intracorneal ring surgery is stop or slow progression of keratoconus, delaying or avoiding the need for corneal transplant surgery. A secondary goal of surgery is to improve vision. Patients will notice a decrease in prescription size and an improvement in visual acuity.

The advantage of corneal ring surgery is that it allows a fast visual recovery without a prolonged recovery time as is needed with a corneal transplant. Rehabilitation time usually takes approximately 3 months.

Nonetheless this surgery does not preclude the possibility of a future transplant surgery. If the intrastromal rings are not tolerated, they can be removed and the cornea will return to its original shape.

  • What risks or complications are associated during or after surgery?

The risk of complications associated with this type of surgery is minimal. Intolerance to the intrastromal ring has been observed in less than 1% of patients. The risk of infection is reported in the medical literature to be 0.0001%.

Is there any risk involved in blepharoplasty surgery?

Blepharoplasty surgery performed by a well trained lid specialist entails minimal risks as with any surgery such as infection or an adverse reaction to anesthetics. Surgical risks can be reduced if instructions given by the surgeon are adhered to. Some minor complications that can occur after surgery include double or blurred vision for a few days, lid swelling on the the temporal side of the lids, slight asymmetry.

After surgery, some patients may find it difficult to close their eyes. Another rare complication is ectropion, which means the lid is rolled out of the eye, which may require another surgery.

The initial evaluation with the surgeon is very important. The lid specialist requires a complete medical history including any allergic reactions, a complete examination and blood work before any surgery can be performed. The ophthalmologist will perform a complete eye exam as well as tear function tests. A frank discussion about objectives and expectations is needed. It is important to discuss if all four lids should be operated (superior and/or inferior lids). It is appropriate to discuss the type of anestesia and sedative that will be used during the procedure.

Where will the surgery be performed?

Clinica de Ojos de Tijuana has three state of the art surgical suites where lid surgery can be performed. An anesthesiologist will be present during the surgery to help monitor the patients vital signs.

Lid surgery is usually performed under local anesthesia.

The Surgery

Blepharoplasty surgery usually takes about three hour depending on the type of surgery and the number of lids to be operated. Blepharoplasty is a typical procedure where the incision are performed along the natural crease lines of the lids making them invisible.

The incision line can be extended to the outer edge of the lid depending on the amount of skin and fat tissue to be resected. If there are fat pads in the lower eyelids, the incision can be performed through the conjunctiva so that there is no scar marks on the skin.

After the Surgery

After the surgery, your surgeon will probably recommend lubricating drops in case of dry eyes. There can light swelling and pain after surgery that is usually alleviated with medication. If pain is severe, please call your surgeon immediately.

Your surgeon may indicate that you maintain elevated for several days and use cold compresses to help reduce swelling and improve coagulation. You will be taught to properly clean your eyes which can have some discharge for a week after surgery. The lid sutures will usually removed between 2 and 7 days after surgery.
